Services Includes:

• Assist with personal care.
• Assist with toileting.
• Assist with dressing.
• Assist with ambulation, including walker, cane, braces, or wheelchair when necessary.
• Perform transfers.
• Remind client to take prescribed medication(s).
• Perform and assist with range of motion exercises.
• Assist with colostomy and Foley catheter care.
• Encourage client to become independent within determined limitations.
• Promote client’s emotional and psychological support to client and/or family.
• Provide emotional and psychological support to client and/or family.
• Prepare meals following special diets when indicated for client and family.
• Perform light housekeeping to maintain client’s environment (areas used by client). Such as vacuuming, mopping and cleaning counter tops. (N0- refrigerator cleaning, cabinet cleaning or dusting.
• Do laundry, washing, drying and folding.
